Developing a National Comprehensive Health Plan for Older People, 2020-2030 – Chile

Summary

The Plan defines as its general objective "Improving the well-being of the older people, through a comprehensive health system that allows them to maintain and improve their state of health and functionality,accessing benefits according to their needs, through coordinated intersectoral work,articulated and integrated care levels, and with the participation of society as a whole.” The initiatives contained in the National Health Plan for the Older people 2020-2030 are operationalized through an action plan that defines objectives, indicators, activities and goals.The different actions are organized into six strategic lines: 1: Education, promotion and prevention; 2: Provision of service; 3: Strengthening of the integrated care network for older people; 4: Strengthening of information systems; 5: Stewardship, regulation and inspection; 6: Financing.

Planning and Implementation
  • What was the challenge you were trying to address?

    The National Comprehensive Health Plan for Older People is part of the Comprehensive Policy on Positive Aging. This policy assumes three strategic objectives, the first of which is to improve or maintain the function of older people, the second is to improve social participation and the third is to improve the subjective well-being of this age group. Two of these objectives are assumed as purposes of this National Plan, these are subjective well-being and social participation. These objectives are assumed as ends due to their multidimensional and intersectoral nature. From this perspective, the Ministry of Health contributes to the achievement of these objectives from its private resources, but its full compliance is outside the limits of sectoral action on its own. On the other hand, the maintenance or improvement of functionality in older people is a health challenge.

  • Please describe how collaboration worked in your initiative.

    Design and construction of a socio-sanitary model for Chile: joint work between the Ministry of Social Development (MIDESO) and the Ministry of Health (MINSAL). - Territorial health interventions in social health network devices. For example, the Digital Health initiative that connects 11 ELEAM of SENAMA, to consultations with doctors and nurses from the Digital Health platform. - Senior Adult Preventive Health Examination Voucher (people over 65 years of age and over). - Technical Committee on Mental Health in Older People (SENAMA-MINSAL): a collective construction on the subject of mental health in older people. Together with the Chilean Society of Geriatrics and Gerontology, a series of recommendations and guidelines were developed, which are detailed below: * Practical guide for self-care of health in older people. * Guidelines for care in home hospitalization of older people in the context of the COVID-19 pandemic. * Recommendations for clinical care teams in the assessment and comprehensive management of older people with a suspected or confirmed diagnosis of COVID-19 at the tertiary level of care.

    Since the plan was published in 2021, we still do not have a response from civil society regarding the impact of the program, however we intend to evaluate the implementation of the plan in 2025.

  • What issues did you face, and how did you address them?

  • What lessons did you learn from implementing this initiative?

    The construction of the National Comprehensive Health Plan for the Elderly and its 2020-2030 Action Plan was based on international recommendations and initiatives such as: the implementation of the 2030 Agenda for Sustainable Development (United Nations Organization), and the Decade for Healthy Aging 2020-2030, serving as a reference framework for its creation. Thanks to the UN Decade of Healthy Ageing, this Plan proposes to create a favorable environment for older people to develop their lives in a healthy way, including themselves in society through their active contribution, achieving personal goals specific to their stage of life, incorporating themselves as managers of initiatives aimed at improving their own quality of life. However, to generate this favorable environment it is also necessary to generate changes at the structural level, impacting the social determinants of health, improving the quality of life throughout the life course, all of which implies working within the health sector, with the intersector, and of course, generating instances of collaboration at the level of the State, civil society and the private world.
