Overview

The integrated care for older people approach, referred to as ICOPE, aims to facilitate the reorientation of health and social services towards more person-centred and coordinated care that supports the optimization of intrinsic capacity and functional ability for older people. ICOPE is WHO’s approach to support the delivery of integrated care for older people within the context of a PHC-oriented health system. This publication, the ICOPE Handbook (second edition), provides a global framework and guidance for the implementation of ICOPE that health workers can use to inform the delivery of integrated care for older people in primary care in their context. It aims to provide flexibility, to ensure feasibility of implementation across different countries, contexts and resource settings. This second edition of the ICOPE handbook in primary care incorporates learning from the implementation of integrated care for older people in diverse contexts around the world and relevant new or updated guidelines produced by WHO.
